The purpose of this form is to collect data which will go to Prof. Keelin O Donoghue to form a report she will be presenting to the HSE National Women and Infants Health Program.
This will be used to direct further research and improve understanding of best practice for medical practitioners who interact with families after a fetal diagnosis. All questions are open ended for you to write out your responses as briefly or as long as you would like. No questions are mandatory, you can skip any question you do not feel comfortable answering or which does not apply to you.
